This form should only be used to submit information / intelligence about specific incidents or crimes. Any information received is checked regularly but not continuously monitored.

Self Administered Interview


If you are a witness to an incident you may be willing to complete this Self Administered Interview form. By filling in this form we will ensure that your memory of the event will be protected and the integrity of the testimony preserved.

You can send completed forms to the Major Incident Team (MIT) at Nexus House, Alexandria Drive, Ashton-Under-Lyne, OL7 0QP, or any police station requesting it to be sent to Nexus House, or hand it to any police officer for delivery.
